Forms for Students Participating in Moore Public Schools Athletics

***For the 2023 - 2024 School Year, Rank One Forms Can Now Be Completed.

***Physicals must be completed by an MD/DO, Nurse Practioner, or Physician's Assistant, no earlier than May 1st of the school year prior to participation.

All student athletes in MPS must complete ALL of the forms on Rank One before they can participate in any sport.
